1st lets state that US Military is the #1 in the World - By far. As is US position in the field of IT sector, from PCs to Operating system to Social network, etc.. So when we say that USA is a 3rd World country, we are referring to Life for average American person on Main Street: from Healthcare to Public Transportation, etc..

In regard to Healthcare:
The U.S. healthcare system is a sick Joke compared to OECD nations, compared to developed Nations and many developing Nations too, with 100Million Americans having no Healthcare at all to being "under insured" to not being able to pay the so called deductibles that average $5,000 per Year . Resulting in USA having the lowest life expectancy, to highest suicide rates , to highest chronic disease/obesity burdens. And on top of delivering much lower result than healthcare in OECD/Developed Nations, US Wall Street based for-profit Healthcare also costs TWICE as much too ! Because USA does not have something as BASIC for good of the People/Nation as Universal Socialized healthcare (USH) that ALL developed Nations have. Because Republicans say that "We cannot afford USH". And because Democrats pretty much say the same thing once in the office. Which utter lies are sold to American People by Big Wall street based Media. Even though USH costs 50% less, repeat costs 50% less, than US for-profit Wall Street based Healthcare.

To public Transportation in USA, or lack of any modern public Transportation in USA of, Trains, Metros, etc.. For example take State of South Carolina, where they have not even connected its Top 3 cities with modern train service, repeat they have NOT EVEN connected its Top 3 cities with modern train service! And the same goes for North Carolina, Georgia, Alabama, etc. US states. In fact the same goes for 95% of US States. OTOH, all European states, or developed Nations and many developing nations too, have connected the top 10 to top 100 cities of their states with modern Train service. For example take State of Austria in Europe, which is pretty much same size as South Carolina (~32,000 square miles), and they have connected the Top 100 cities in Austria with modern train service. To China and European countries having 50,000 Miles of High speed train, while USA has almost Zero miles of High speed train. Now to put things in clear prospective, modern Train Transportation is most intelligent way for People to Travel, as it is environmentally clean since it is electric powered, it allows people to socialize rather than be isolated in their cars, frees people from cost of owning a car and hence being at mercy of Big Oil & Big Auto, and it means SAVING 10s of 1 000s of lives per Year. Repeat means SAVING 10s of 1000s of lives per Year, which is why USA has a death in Highway rate that is 10 times worse of Europe while Europe has a population 200 Mill more than USA .

And on just about every other aspect of Life for People on Main street USA is totally lacking behind developed Nations:

1- Unlike all developed Nations USA does not have Paid (socialized) maternity leave

2- Unlike all developed Nations USA does not have Paid (socialized) day care

3- Unlike all developed Nations USA does not have Paid (socialized) vacation days

Because Wall Street Billionaires, via their control of Media, and hence all Republicans and most Democrats too, have brain-washed American People "that we cannot afford it", even though US supposedly is the Richest country in the World, and even though Socialized healthcare costs 50% LESS, repeat LESS, than based for-profit based Healthcare US has.

OTOH, Wall Street Billionaires in USA are spending 100s of Billions of Dollars on AI, in fact close to $1.5-Trillion on AI, such as: $150-Bill to date into OpenAI, $65-Bill into Anthropic, etc. with OpenAI alone announcing a mind-bugling $110-Billion funding today , which is an amount greater than GDP of 100 countries in the World! When with this same $110-Billion given to OpenAI they could have built something as essential as modern Train service to connect at least the top 3 Cities of 10 US States, give Paid maternity leave to Women in USA as all developed Nations do, etc..

So what is going on that USA is a 3rd World country when it comes to things as essential as Healthcare, Transportation, Paid maternity leave, etc.; while spending 100s of Billions, Trillions of Dollars, on something as non-essential as so called AI? What is going on here?!?
